   We are pleased to inform you that effective 10-13-05, your Prescription Drug Discount Program that is included with your Platinum Benefit Group membership will be provided by Agelity Drug Discount. Agelity is a leader in the Pharmacy Benefit Service industry with well over 53,000 participating pharmacies. With this new provider you will enjoy discounts on your prescription drugs at no additional cost to you.

 

To use this valuable benefit simply:

  1. Locate a pharmacy in your area by visiting www.platinumbenefitgroup.com or call 1-877-344-4835.
  2. Visit the pharmacist in your area and have him call 1-800-847-7147 to verify eligibility
  3. Give your ID numbers

 

Again effective 10-13-05, you should begin using the above phone numbers and identification numbers when purchasing prescription drugs instead of calling the Catalyst Rx phone number as listed on the back of your Platinum Benefit Group MasterCard.

You are already enrolled! You will not be charged any additional cost for this program.
